Services: * Family planning clinics include free and reduced cost birth control, pregnancy testing, and family planning education. Costs are based on sliding fee scale and most methods of birth control are available.

Health Care

Services: *Women, Infants, and Children (WIC) Program: Provides nutritional education and supplemental food coupons to pregnant and postpartum women, infants and children (up to age five) who meet financial and medical requirements.
*Immunizations: *Vaccine administration services available through clinics and individual appointments.
*Fee: based on ability to pay.
*Medicaid Health Screening (EPSDT):*Health screening and counseling are provided to persons under 21 years of age in collaboration with local physicians. Transportation may be provided.
*HIV and AIDS Counseling and Testing: also pre-marital counseling as required by Michigan law.
*Reproductive Health: *Distribution of contraceptive methods, diagnosis and treatment of sexually transmitted diseases, health counseling and medical referrals as needed.
*Children's Special Health Care (Crippled Children): *Specialty medical care services are available to children with potentially handicapping conditions and/or chronic illness. Services include diagnostic and medical care nursing assessment and case management, home care, equipment and specialty clinics.
*Mich-Care: *Full Medicaid coverage is available to women for their pregnancy and through two months postpartum. Assistance in making application for MICH-CARE is provided, via personal appointment.
*Breast and Cervical Cancer: *Confidential health counseling, screening to women over age 40, who meet financial guidelines. *Clinical breast exam, pap smear test with referral for mammography and other health care needs. *Annual follow up services are provided.
Community Services:
*Heart Health Screening:*Blood Pressure and cholesterol screening, monitoring, counseling and referral are conducted in scheduled clinics and by individual appointment.
*Maternal and Child Health Programs: *Pregnant women who have Medicaid are eligible to receive services which addresses psychosocial, nutritional, non-medical care, as well as transportation for health care. Infants of Medicaid eligible women and other identified high-risk families are eligible for more specialized case management, provide primarily through home visits. Prenatal classes are provided quarterly in collaboration with other health care providers.
*Hearing and Vision Program:
*Communicable Disease: *Disease reports are analyzed casefinding is provided, and disease prevention information is shared to reduce the spread of preventable communicable diseases. TB tests are available.
*Senior Screening:
*Part H Early On:*Multi-agency interdisciplinary team of professionals who provide family centered care to identify strengths and needs. Families are provided assistance in securing resources for appropriate follow-up. Wrap around services available for special families.

*Blood Lead Home Assessment:*Confidential health education/counseling services to follow-up on blood lead results that fall outside the normal range.
Home Health Care Services: *Evaluation Home Visits:*Home Visits: *Services are provided by nurses, social workers, home health aides and therapists in the home.
*Private Duty: *For persons who need assistance to meet their personal care needs can request this specialty service.
*Diabetes Education
*Foot Care Clinics
*Coordination/Liaison with other Agencies
Hospice Care: *Support and care are provided to terminally ill clients and their families in the client's personal surroundings.
Environmental Health Services:
*Food Service: *Program includes licensing, plan review, inspection construction and disease investigations of facilities serving food to the public.
*Onsite Sewage Permits: permits for sewage systems, *Well Permits, *Environmental Complaints
*Mortgage evaluations (water and sewage evaluations), Land Evaluations, *Public Swimming Pools
*Subdivisions-site Condominiums, *Campground Inspections
*Family Independence Agency Licensed Facilities: Foster homes, day care centers, and children's camps
*Septic Waste Haulers: *Licensing and annual inspections, *Mobile Home Park Licensing, *Ground water Education (GEM), *Soil Erosion & Sedimentation, *Indoor Air Quality, *Radon
Health Education Services:
*Aids Education, *Community Health Promotion, *Heart Health Education, *School Health Education, *Worksite and Community Health Promotion, *Addiction Services
*Skilled Nursing: Catheter care, ostomy care, medication teaching, injections, teaching patient and caregivers, observation and evaluation, blood pressure, blood glucose testing, I.V. medication instillation, I.V. care, diabetes teaching and care, diet teaching.
*Home Health Aide: Personal care such as bathing, shampoos, meal preparation, feeding of patient, respite care, assistance with ambulating and exercises.
*Medical Social Work: Assessment of social and emotional factors affecting illness, community resource coordination, counseling for long-range planning and decision making.
*Occupational, Physical, Speech Therapy, Registered Dietitian & Private-Duty Services also available.

Services: *Every Hill-Burton hospital can choose which types of services to provide free. They do this in an "Allocation Plan"
*The hospital must give you a written "Individual Notice" which will tell you what types of services it is providing.
*Hospitals costs only are covered, not your private doctor's bills.
*Hospitals must give out a certain amount of free care each year, but can stop once they have given that amount.
*You have to ask to get these services
*You can ask for free care at any time, even after you've received services.
*To get a list of Hill-Burton hospitals in your area, write to the address listed above, or call the Hot Line number.
Eligibility:
*If your income is less than the CSA Income Poverty Guidelines, hospital services can be free.
*If your income is greater than, but no more than double the CSA Guidelines, Hill-Burton hospital may give full charge, reduced charge, or free services.
*The hospital may ask you for proof of your income, such as a pay slip or and income tax form.
*The hospital's "Individual Notice" will tell you what income levels qualify for free charge.
*Hill-Burton hospitals have two working days to determine your eligibility.

*The hospital must provide you with a written statement which says either when you can get free services or why you've been denied.
*The hospital may deny your application for certain reasons including...
*If your income is more than twice the CSA Guidelines.
*If the hospital has given out its required amount of free care.
*If the service you requested or received are not covered under the hospital's "Allocation Plan", which is published in the newspaper.
*If the services you requested or received will be paid by Medicare/Medicaid or other insurance.

*You may file a complaint with your Department of Health and Human Services Regional Office if you think you have been unfairly denied services.
