SERVICES: SENIOR CITIZENS PROGRAMMING - These programs perform
planning, service coordination and advocacy for senior citizens and serve
as community focal points, along with the senior citizens, for information
for and about older citizens. Direct services include information and
referral, outreach to isolated persons, health screening and tax assistance.
They also provide homemaker aide, transportation, counseling, recreation,
respite care, personal care, Meals on Wheels and congregate nutrition
meals through fifteen senior centers in the U.P.

NEWSPAPER: The "Communicator" sent to area seniors
SENIOR CITIZEN/HANDICAPPED IN-HOME SERVICE PRO: Comprehensive
functional assessment of persons aged 60 and older who are at risk of
institutionalization. Arranges and schedules existing community based
health, social services and possible support systems.

entry to access information, resources and assistance when navigating the long-term care system.
. Utilizing the assistance of a UP Long-Term Care Connection Options Counselor,
the consumer will be provided with the entire array of options available based on their particular circumstance.
The entire process is centered upon a principal of ensuring that individuals are provided with timely, unbiased and
